Your Medical Records
You can access your medical records online any time through our patient portal. Your patient portal account includes test results and other details about your medical care.
Who Can Request Medical Records?
You can request your own hospital medical records and those of a child in your care who is under 18 years old.
Others who can access medical records include:
- Caregivers with power of attorney
- Estate executors of deceased patients (executor paperwork must accompany the record request form)
To Obtain Medical Records from Joslin Diabetes Center and BIDMC Joslin Diabetes
Medical records are available for 20 years after the patient's discharge or final treatment. To obtain a copy of a medical record, an authorization to release protected health information must be properly filled out.
- Upload a letter of request and a completed authorization to release protected health information to the Requestor Submission Portal* OR
- Fax a letter of request and a completed authorization to release protected health information to 866-710-0132.

Do not upload/fax subpoenas or court orders; all subpoenas and court orders must be submitted to:

Records will be delivered to the email, fax or mailing address specified on the authorization. When faxing the authorization form, please include the Joslin Fax Cover Sheet. Patient or Patient’s Legal Guardian
We reserve the right to charge a reasonable cost-based fee for producing and mailing the copies, pursuant to HIPAA 45 CFR,164.524. This rate depends on the size and delivery method of the records being requested. At no time will the cost-based fee exceed Massachusetts law (MGL Chapter 111; Section 70).
The fee structure follows Massachusetts law (MGL Chapter 111; Section 70).
To request copies of X-ray, MRI, nuclear medicine or other imaging, please follow the Department of Radiology instructions under the "Obtaining Copies of Your Images" section.
For certified copies, please contact the City of Boston, Registry Division.
