Your Medical Records

You can access your medical records online any time through our patient portal. Your patient portal account includes test results and other details about your medical care.

Who Can Request Medical Records?

  • Patients (requesting their own medical records)
  • Parents/legal guardians of children under 18 years old
  • Caregivers with power of attorney
  • Estate executors of deceased patients (executor paperwork must accompany the record request form)

Requesting Records from Joslin Diabetes Center and BIDMC Joslin Diabetes

Medical records are available for 20 years after the patient's discharge or final treatment. To obtain a copy of a medical record, an authorization to release protected health information must be properly filled out.

Fees for Medical Records

Patients or Legal Guardians

We reserve the right to charge a reasonable, cost-based fee for producing and mailing copies of medical records, in accordance with HIPAA 45 CFR,164.524.

The exact fee depends on the volume of records and the delivery method requested. At no time will the fee exceed the limits set by Massachusetts law (MGL Chapter 111; Section 70).

The fee structure follows Massachusetts law (MGL Chapter 111; Section 70).

Medical Imaging (X-ray, MRI, etc.)

To request copies of X-ray, MRI, nuclear medicine or other imaging, please follow the Department of Radiology instructions under the "Obtaining Copies of Your Images" section.
