Concord Health Care Center, MA Be part of an innovative new Joslin Diabetes Center program. Concord Health Care Center in Concord, MA is collaborating with Joslin Diabetes Center to improve diabetes care in their skilled nursing facility. The Diabetes Care Coordinator is a new 32 hour nursing position at Concord responsible for diabetes staff training, patient education and quality improvement initiatives. Certification as a diabetes educator is preferred but not required. Site Manager Position, responsible for supervising, directing and scheduling the personnel within the OSFMG Endocrinology office and Joslin Diabetes Center. In conjunction with the Medical Director, evaluates the overall performance of individuals towards delivery of high quality medical care and effective business operations. Works towards the smooth integration of functions of the front desk office staff, the clinical staff, and the ancillary staff, to assure coordination of efforts and maximum productivity. Responsible for recommending, implementing, and administering budgets, policies, and procedures for the practice(s). Maintains cooperative working relationships with physicians, midlevel providers, OSFMG Site/ Project Managers, Medical Center personnel, other OSF entities, and external agencies and organizations. Plans, organizes and develops the activities for the Joslin Diabetes Center program.
